Yes. Homemaker, companion, respite, transportation, personal support, and community participation services can all be combined into a single individualized plan.
Yes, we work with case managers on Minnesota HCBS waiver funding for eligible individuals, in addition to private-pay options for Basic Home Care.
We start with an assessment of the individual's needs and goals, then build a person-centered plan in partnership with the individual, their family, and case manager where applicable.
It means your care plan is built around your specific goals, routines, and preferences, rather than a one-size-fits-all template. We work with you (and your family or case manager) to design support that fits your life.
Yes. Care plans are reviewed and adjusted as needs evolve, whether that means adding services, changing hours, or shifting the type of support provided.
Payment & Eligibility
We work with CADI, Elderly Waiver (EW), and Brain Injury (BI) waiver funding, as well as private pay. We'll discuss which options apply to your situation during your first conversation with our team.
Yes. 245D Home and Community-Based Services are accessed through a Minnesota county case manager or care coordinator, who helps determine waiver eligibility. We coordinate directly with your case manager throughout the process.
No — an initial consultation to discuss your needs is free, with no obligation.
Basic Home Care services are also available as private pay for individuals who don't qualify for, or aren't pursuing, waiver funding.
Getting Started & Referrals
Anyone can submit a referral — hospitals, case managers, social workers, healthcare professionals, and family members are all welcome to reach out on behalf of someone who could benefit from care.
Our team reviews the referral and reaches out to schedule an assessment or consultation. If 245D waiver funding is involved, we coordinate with your case manager to help finalize an individualized care plan.
Timelines vary based on the assessment process and, where applicable, waiver approval — but we begin working on next steps as soon as a referral is received.
Absolutely. Person-centered care plans are built with input from the individual and, when appropriate, their family or support team.
Caregivers & Safety
Yes. Our caregivers undergo background checks and training consistent with Minnesota home care licensing requirements before working with clients.
We prioritize consistency and try to match clients with caregivers based on needs, personality, and schedule wherever possible.
