Yes, we work with case managers on Minnesota HCBS waiver funding for eligible individuals, in addition to private-pay options for Basic Home Care.

We start with an assessment of the individual's needs and goals, then build a person-centered plan in partnership with the individual, their family, and case manager where applicable.

Yes. 245D Home and Community-Based Services are accessed through a Minnesota county case manager or care coordinator, who helps determine waiver eligibility. We coordinate directly with your case manager throughout the process.

Basic Home Care services are also available as private pay for individuals who don't qualify for, or aren't pursuing, waiver funding.

Anyone can submit a referral — hospitals, case managers, social workers, healthcare professionals, and family members are all welcome to reach out on behalf of someone who could benefit from care.

Our team reviews the referral and reaches out to schedule an assessment or consultation. If 245D waiver funding is involved, we coordinate with your case manager to help finalize an individualized care plan.

Timelines vary based on the assessment process and, where applicable, waiver approval — but we begin working on next steps as soon as a referral is received.
