How 245D Services Begin
Eligibility & Waiver
Eligibility is determined through Minnesota's HCBS waiver programs, typically coordinated with your case manager.
Referral
A case manager, social worker, hospital, or family member refers the individual to BrightFuture.
Individualized Plan
We build a Coordinated Service and Support Plan (CSSP) addendum around the person's specific goals.
Care Begins
A matched, trained caregiver begins delivering consistent, individualized support.
