Individualized Home Supports (IHS)

245D is Minnesota's licensing framework for Home and Community-Based Services (HCBS) — designed to help individuals with disabilities and complex needs live safely and independently in their own homes and communities.

Getting Started

How 245D Services Begin

Eligibility & Waiver

Eligibility is determined through Minnesota's HCBS waiver programs, typically coordinated with your case manager.

Referral

A case manager, social worker, hospital, or family member refers the individual to BrightFuture.

Individualized Plan

We build a Coordinated Service and Support Plan (CSSP) addendum around the person's specific goals.

Care Begins

A matched, trained caregiver begins delivering consistent, individualized support.
